The footage opens in a quaint, fog-shrouded town, only to twist into a nightmare of twitching, malformed creatures and graphic dismemberment. What begins as a seemingly peaceful exploration quickly spirals into an unrelenting gauntlet of grotesque encounters, each more visceral than the last.
The level of detail in the dismemberment system—bones snapping, flesh peeling, and blood spattering—underscores Mundfish’s ambition to push horror boundaries.
Viewers noted that even after the trailer appears to end, a sudden “false finish” plunges the audience into a fresh wave of terror, demonstrating a keen understanding of tension and release in horror pacing.
Developers have hinted that these mechanics serve not just shock value but tie into a narrative about corporeal decay and the fragility of the human body when confronted with otherworldly forces.

Fans of atomic-level horror—the same studio behind Atomic Heart—have high hopes that ILL will balance its punishing visuals with thoughtful level design and a gripping story.
During the event, Mundfish also teased Atomic Heart 2 and The Cube, but ILL stole the spotlight with its unapologetic intensity.
